Planning & Palliative Care = TLC:

Collaborating with healthcare professionals to compliment care for those with advanced illness.

What does TLC do?

  • Provide in depth assistance with Advance Care Planning for yourself, or a loved one with a chronic or advanced disease condition.
  • Palliative Care seeks to prevent, relieve, reduce or soothe the symptoms of disease or disorder without effecting a cure…it is not restricted to those who are dying. It attends closely to the emotional, spiritual, and practical needs and goals of persons and those close to them. Palliative Care focuses on qulaity of life, comfort and choices.

Who provides the care?

  • TLC is an interdisciplinary team, with physician advisors, certified nurses, our Chaplain, and Social Workers.
  • A Palliative Care certified nurse, in coordination with the Chaplain and the Social Worker will assess the needs of the person and the family in regard to symptom management, psychosocial concerns, and spiritual needs. The team works closely with Care-Management, Hospice, and Home Health. You and your primary healthcare provider make all healthcare desisions.

Who can benefit from the program?

  • TLC is designed to assist persons with chronic or advanced illness in completing advanced directives.
  • TLC staff can provide support and information for persons with advanced, life-threatening illness and its complex issues that may still be actively seeking treatment.
  • It is a “safety net” for persons seriously ill that may not be ready for hospice.
  • TLC can assist persons desiring to discuss end-of-life issues
  • The TLC team can provide care for persons who are near death in the in-patient setting.

Goals of TLC:

  • Promote Advanced Care Planning
  • Collaborate with the primary healthcare providers to meet physical, emotional and spiritual needs of persons with advanced life-threatening illness.
  • Provide seamless support across the continuum of care for advanced illnesses.
  • Educate healthcare providers, patients, families and the community regarding palliative care.
  • Facilitate meaningful interactions between families and healthcare providers around goals, needs, concerns and final wishes.
  • Optimal use of healthcare resources.

Transitions and Life Choices
(TLC) reports to Care Management and Homelink. Messages can be left at 447-2533. Care and Consultation is available 8-4, Monday through Friday at St. Peter’s Hospital. Phone follow up consultation is available on an as needed basis.
